What Does "Permanent & Total" Mean?
VA Permanent & Total (P&T) disability status is the highest protection level the VA grants. "Permanent" means the VA has determined your service-connected condition(s) are unlikely to improve — there's no reasonable expectation of recovery. "Total" means your combined disability rating reaches 100%.
P&T status does two important things: it protects your rating from reduction (the VA will not schedule future re-examinations) and it unlocks a suite of additional benefits for you and your dependents that are not available to veterans with standard 100% ratings.

Complete List of P&T Benefits
1. CHAMPVA (Civilian Health and Medical Program of the VA)
CHAMPVA provides comprehensive health insurance coverage for eligible dependents of P&T veterans. It covers doctor visits, hospital stays, surgery, mental health care, and prescription drugs. Eligible dependents include your spouse and children under age 23 (if enrolled in school). CHAMPVA is coordinated with Medicare or other insurance when applicable.
2. Fry Scholarship (Marine Gunnery Sergeant John David Fry Scholarship)
Children of P&T veterans are eligible for the Fry Scholarship, which provides the same benefits as the Post-9/11 GI Bill: up to 36 months of tuition payments at the in-state rate, a monthly housing allowance, and a books and supplies stipend. Spouses are also eligible in some circumstances. Children can use this benefit until age 33.
3. State Property Tax Exemptions

4. Commissary, Exchange, and MWR Access
P&T veterans and their dependents have full access to military commissaries (grocery stores), exchanges (retail stores), and Morale, Welfare, and Recreation (MWR) facilities on military bases. This can provide significant savings on groceries and everyday purchases.
5. VA Life Insurance (VALife)
Veterans with service-connected disabilities, including P&T status, may qualify for Veterans Affairs Life Insurance (VALife) — up to $40,000 in whole life coverage regardless of health status, at competitive rates.
6. State Vehicle and Income Tax Exemptions

How to Get P&T Status
Automatic Designation
Some veterans receive P&T automatically when the VA makes its rating decision. This happens when the rating is 100% and the VA's examination evidence already supports permanency. You'll see language like "Permanent and Total" or "no future examinations scheduled" on your decision letter or ebenefits/VA.gov profile.
Requesting P&T Designation
If your decision letter shows 100% but doesn't explicitly state P&T, you can request a formal P&T designation by:
- Submitting a written request to your VA Regional Office
- Including a medical statement from your treating physician(s) stating your condition(s) are permanent and will not substantially improve
- Getting a private medical opinion (nexus letter) supporting permanency
- Having your VSO submit the request on your behalf
TDIU + P&T
Veterans rated 100% under TDIU (Individual Unemployability) can also receive P&T status. Once you have P&T status, your rating is strongly protected. The VA can only reduce a P&T rating under very limited circumstances, and reductions require specific procedures including advance notice and a hearing. Unlike regular ratings, P&T ratings cannot be reduced simply because a C&P exam shows improvement — the VA would need to show sustained improvement in an ordinary life setting.
